I was recently rejected for a renewable four year scholarship. Their reasoning was that I didn't include my transcript, but I have a reliable witness who can claim that I included it in the envelope. I've seen multiple formatting for appeals, but I'm at a standstill because most, if not all, are for those that do not meet grade requirements. It's not the greatest formatting, but I'm looking for corrections and information I can include to help my case. Thanks in advance!

Moreover, she has already sent you a letter explaining her side of the story. Anyway, I am attaching a second copy of the transcript with this letter. I, therefore, request you to please consider me for this scholarship as I have put in a tremendous amount of effort to meet the eligibility requirements of the scholarship.
