Correction of sentence written in school declaration
Hello guys! I need your help...

I am not fluent in English and I would like to know if I wrote the two sentences below correctly and how do I put the date, city and country at the end of a declaration that must be signed by the principal.

1) Official certificate proving that the student Maria Eduarda Carvalho met the standards established by TEKAI in relation to her academic achievements.

2) High school grades converted from a 100-point grading scale to a 4.0 point grading scale (G.P.A).
