Grammar, Usage / A mature student who hasn't written an essay in 20 years needs help [2]

I don't know whether this post should be in this section, so please forgive if it is not.

I am a mature student that hasn't written an essay in 20 years so have forgotton a few things. i have written the piece I need but am unsure whether a) I need to out the question in with the essay at the top or if the essay just begins with my description, b) do i need to put my name on each page or does this just go onto the seperate piece i hand in given to me from my college.

If anybody could help me would be great I have tried to find these answers online but to no avail.
